We advise all patients to verify coverage with their insurance provider as any and all financial liability ultimately is the patient’s responsibility.

If your claim is denied, you will be responsible for charges as a result of the claim. If you have questions, please contact our billing office staff at 866.263.8525.

In the event we don’t participate with your health insurance plan, you may be able to see one of our physicians using your out-of-network plan. Our insurance coordinator specialists in each of our centers will be able to assist you with your questions regarding benefit coverage for your health insurance plan.

Our insurance coordinator specialists are also able to assist you with your questions regarding co-payments, annual deductibles and out-of-network benefits as well as questions about durable medical equipment and prescription medications. However, each insurance carrier offers several different benefit plans.

We encourage you to obtain complete information from your health insurance administrator before making health care decisions based on insurance coverage.
