Save Time in Advance of Your First Visit…

Register with our Patient Portal to complete and submit the required New Patient Questionnaire and Signature documents. For your first office visit with National Spine & Pain Centers, please bring the following with you:

  • Photo identification
  • Health insurance card
  • Referral from your referring physician
  • It is your responsibility to check with your insurance company to determine if a referral is required
  • X-ray, MRI, any test result, prescriptions
  • Or additional information that would be helpful in diagnosing and treating your pain

Kindly arrive at least 30 minutes prior to your scheduled appointment time in order to complete your registration and any other paperwork that may be required.

During your first office visit, you can expect a thorough evaluation by your physician. Some patients will be scheduled for a follow-up procedure and be given specific information and instructions regarding preparation for the next appointment.
