Workers’ Compensation

Workers’ Compensation claims must be authorized by your employer. At the time of your appointment, please provide the following:

  • Workers’ Compensation claim number
  • Date of injury
  • Claim forms
  • Names, addresses, and telephone numbers of your employer, your immediate supervisor’s name and the Workers’ Compensation insurance carrier

If your claim is denied, you will be responsible for charges as a result of the claim. If you have questions, please contact our billing office staff at 866.263.8525.
