Existing Patient Prepare for Your Upcoming Appointment
We appreciate your trust in NSPC for your care. To ensure your follow-up goes as smoothly as possible, please follow the steps below!

    Schedule an Appointment

    For your convienance, we offer three ways to schedule a follow-up appointment:

    1. Schedule your follow-up prior to leaving the practice! Speak with our staff at check-out.
    2. Call us directly at the phone number provided for a specific physician or location.
    3. Submit an appointment request online, and we'll call you back within 1-business day.

    Insurance Validation

    Upon scheduling your appointment our team will complete an insurance authorization, ensuring that your insurance carrier is accpeted, and if not, reaching out to you directly to determine the next best course of action as it pertains to your care.

    Updates to Care Plan

    If you've had any updates to your care plan, now is a great time to update these changes in our system. You may submit these notes directly via the patient port link here (please login with your registered credentials). If you answer "yes" to any of the below, feel free to send us a note directly, via the patient portal. If you're having trouble accessing the portal, please feel free to contact the practice directly, and we'd be more than happy to help walk you through it.

    • Have you had any changes in care since your last visit with an NSPC affililated physician?
    • Have you had any changes to your medications?
    • Do you have any imaging that you need to include for your upcoming appointment?

    Complete Pre-Appointment Questionnaire

    In an effort to provide the best care, and a clear understanding of the pain that you're currently coping with, if anything has change following your previous appointment, please complete the below questionnaire. This helps our team best prepare for your upcoming appointment, and also ensures that you receive the most suitable therapy.

    Appointment Reminder


    X days in advance of your appointment, you will receive an appointment reminder, this may come via SMS text message, email, or both!

    This is a great opportunity to make sure you've completed your pre-appointment paperwork. If you haven't already, no worries, simply utilize the links below, and complete it prior to your visit! (links to both packets):

    What to expect on your appointment day:

    • Please plan to arrive 15-30 minutes ahead of your scheduled appointment time.
    • Bring a book or reading material in case you get to the office in advance of your appointment.
