Your First Visit

Thank you for choosing SOAR as your pain management provider.

Please arrive 30 minutes prior to your scheduled appointment time and ensure you read the below helpful information so that your visit is a positive one!

  1. For your initial visit, please bring with you your current insurance cards (primary, secondary and tertiary) and a photo identification, such as your driver’s license or social security card. It is required that these items be shared at the time of your check-in, and failure to provide this identification will result in cancellation.
  2. If your PCP (primary care physician) referred you to our office, please contact them to request they submit your medical records to our office, including any imaging reports. If you are a self-referred patient, please ensure you obtain those medical records pertaining to your pain and either bring them to your first appointment or have them faxed to (321) 733-7970.
  3. If your insurance requires authorization for any office visits including your initial consultation, it is your responsibility to obtain that authorization. Please speak with your Primary Care Physician regarding this process and while doing so, ensure that they have your correct insurance information when referring you to our facility. If an authorization is required and not obtained, you may incur fees from your visits.
  4. There may come a time when you require additional medical and/or insurance forms to be completed by our office. They may include, but are not limited to, Disability Forms, Workers’ Compensation Forms, Attending Physician Statement, Leave of Absence forms, etc. This will not apply to most patients. In order to accommodate these requests, it will necessitate reviewing the chart, staff time and office resources. Therefore, a reasonable fee of $15 for such services will be applied and must be received before forms will be completed. Every effort will be made to have these forms completed within a 5-7 business day turn-around from the time the fee is received.
