Capitol Spine & Pain Centers ("CSPC") respects the privacy of users of our web site. This Privacy Statement explains our online information practices regarding the collection of personally identifiable information. Personally identifiable information is information, such as your name, physical address, e-mail address, telephone number and medical information. If you provide us with personally identifiable information, we will treat it in accordance with this Policy. Information Collection and Use

There are three areas of the CSPC web site that require users to provide personally identifiable information:

You may choose at any time not to provide personally identifiable information to us, although this may restrict our ability to provide requested services or information. CSPC uses the personally identifiable information collected from you for the purposes for which it was submitted. We may also use your information to provide you with better service and communicate back to you. With your specific permission or written authorization to the extent the testimonial includes protected health information; we may share your testimonials with other future patients on our web site.

Web Site Technology

  • Non-Personally Identifiable Information. CSPC’s web server logs information that does not, by itself, identify a specific individual. We collect certain information about the use of our web site by our visitors. This information is then compiled and analyzed in a way that does not identify our visitors personally. The information may include the Uniform Resource Locator (URL), indicating from where a visitor to our web site has come, which URLs have been visited on our web site and in what order, the date and time of your visit, what browser is being used and what Internet Protocol (IP) address a visitor used to get to our web site. We use this information for web site and system administration to improve our service.

  • Linked Web Sites. We may provide links to third party web sites. You acknowledge that such links are provided for your convenience only and do not constitute an endorsement by CSPC of such third party web sites. We do not control those web sites and assume no responsibility for the privacy practices of those web sites. We encourage you to review the privacy policies posted on those, and all third party web sites.

  • Cookies. Our web site uses Cookies, pieces of numeric data that we transfer to be stored on your computer so that we can easily identify your computer when you return to our web site. CSPC can only “read” Cookies that are placed on your computer from our web site. Usage of Cookies is not linked to any personally identifiable information. You can choose to disable Cookies by following the procedure suggested by your particular browser. If you choose to disable Cookies in your browser, you may still be able to access portions of our web site content, but some of the functionality of the web site may be impaired.

Protecting Your Information

We provide reasonable protections for the personally identifiable information you share with us. Unless we have your permission or are required or permitted by law, we will only share the personally identifiable information you provide with authorized CSPC staff and third party service providers that perform services on our behalf, all of whom are required to keep your information confidential. CSPC does not sell, loan, trade or lease any personally identifiable information collected on our web site to unrelated third parties for their own purposes.

For the services requiring personally identifiable information, CSPC uses a technology called Secure Socket Layer (SSL). If your browser is capable of SSL (and most are), your personally identifiable information will be protected during transport across the internet.

We do not knowingly collect personally identifiable information from children under the age of 18. If we learn that we have personally identifiable information on a child under the age of 18 we will delete that information.

Changes to the CSPC Policy

CSPC reserves the right, in its sole discretion, to modify, alter or otherwise update this policy at any time and you agree to be bound by such modifications, alterations or updates. We will notify you of material updates to this privacy policy by posting the revised policy, with the date it was revised on this page.
